Broken fulcrum, bent pushrod, all hell has broken loose

Ok chicken or the egg type deal here.

First off I have never ever seen this on a 7.3. #7 had an ever so slight misfire and an obnormal crank cadence. I just put UVCH on last month, figured one of them let go already but I found this.

I can get a pry bar on the intake valve and hit it with a hammer and can hear the valve open, but exhaust I can't get a good angle on it.

Truck was driven to me over 100 miles like this so I can't imagine what was happening to all the fuel in the cylinder...

I can't look at the lifters good, I didn't bring me borescope home...

Any ideas?

Swapped the rocker and good pushrod to both sides and both valves move freely.. Pending lifter inspection I'm just going to throw a good pushrod and fulcrum in and see what she does

And no its not a manual so chances are over Rev didn't cause this

I have seen the rockers crack around the ball bearing cup and cause the valve not to open but never seen a mount break like that.

Did the intake or exhaust break?

I would think that it should be fine. The one that was brought to me was run for weeks with it popping out the intake and after changing the 2 rockers it ran fine. Still running now and that is over a year ago.

Maybe just plain wore out?
Don't hear much of this on a stock, or well setup engine. Usually bent pushrods.

Who knows whose driving it at any day. Plow and work horse.

Nope. Starts great bone cold and hot. Was running down highway when this happened

Sometimes it just happens. Maybe a rocker was loose and you didn't notice it when you changed the UVCH (not looking for issues). Swap everything out and see what it does.

The straight pushrod at the top of the tip has very weird surface erosion.
