Cant keep needle bearings in my truck!!!

Hey guys I rebuilt my front axle shafts just after christmas, havent touched 4x4 yet as I have been procrastinating on sending my hubs back for warranty replacement (warn) yes... That lazy, anyways I was driving the other day and hit a pothole, front end started vibrating, so I crawled underneath to find my long shaft once again sloppy, I dont understand if im doing something wrong or what but I cannot keep these parts for more than six months ive pressed everything on properly, everything is sealed and installed. After christmas I did, d side wheel bearing hub seal, axle seal and u joint, on p side I did a needle bearing hub o ring hub seal and axle seal... No clue where im going wrong here!!! Help!

is the shaft where the needle bearing rides grooved? I had to replace my entire shaft due to this... FWIW, ebay had the entire shaft, joint, knuckle, and seals for cheaper than the u joint and seals sold separately.. all pressed together ready to rock
