Engine tick while in drvie

2011, with 125k miles. The truck has a ticking sound when its at idle in drive or if you rev the motor, when the rpms are coming down then it ticks. If its just idling in park there is no tick. Any ideas? Its ******d and i have noticed a small puff of smoke now on take off. No codes.
