Was loosing coolant out of the main reservoir on my truck and for the first few minutes after start up it would smoke like a freight train. Checked all the common places for leaks (turbo return line, radiator). I did find a leak on the thermostat housing and fixed that but that did not help the issue. Was just adding coolant while trying to find the issue until I cranked it up one morning and noticed a "miss" and the truck would shake around 1,200-1,500 rpm. At that point I threw up my hands and sent it to my mechanic. After letting it sit for 2 weeks he tried to crank it up and it hydrolocked. Once he started digging he found that the #4 cylinder had coolant in it (shooting out the glow plug hole when turning it over). He pulled the valve cover/intake after putting pressure on the coolant system for some time and coolant had pooled in the head. From that I gather that there was a casting defect in the head.
