Most F250 pickups don't come with the helper spring because it's included and only available with the "camper package". The camper package also includes a rear stabilizer bar and heavy duty front springs. F350's come with the helper spring by default, but you gain the stabilizer bar if you add the camper package.

Also, Ford released a TSB a while back to address some complaints with the rear suspension on the F250's. Basically, it was a parts list for guys that wanted the rear of the truck to sit higher. (That only applies to certain model years though, but the bottom line is that it's definitely possible to use factory components to attain heavier-duty suspension.)

Found a F250 with the Camper and Snow Plow package with the taller 3 3/4" rear block the other day.

Just depends on what you want/need the door sticker to say. From the factory there can be a few physical differences but the door sticker is the primary difference for what matters.

My 250 does not have the camper package, but does have the snow plow prep package. It also came with the 3 3/4 block in the rear. The back end of my new 250 sits as high as my 2011 350 did.

2011-2013 F250's with plow prep will have a 2.0" rear block.
2014-2016 F250's with plow prep will have a 4.0" rear block.

A lot of customers complained about the rear ride height on 2011-2013 trucks, so Ford released a TSB with the required parts to convert to 4.0" block. To do the job correctly, the driveshaft and a couple other parts all need to be changed out. Unfortunately, Ford doesn't cover this under warranty...
