Never heard of burning a hole in a piston.

Happens all the time - can be caused by many things - but usually a crack that gives away just enough that it begins to get a hot spot, melt, and then when it gets thin enough - its like a blow torch cutting through the piston and sending piston bits into the crankcase. Its not seen as often in 7.3s as other motors, but it does happen. Its quite common in many of the common rail motors.
