94 overheated

I'm a 6.0 guy so I'm calling in you 7.3 boys for some opinions. My uncle has a 94, unsure of the mileage, mint condition. He has babied that truck since new. Said the other day it overheated...fan clutch went out. Now, once ect gets up and opens the tstat, it starts pushing coolant out the overflow tank. The way he talks it's pushing a old bit out, just sitting there idling. But only does it once the tstat opens. My first thoughts were blown HG/cracked it warped head. But if that were the case, why only once temps are up? If there's that much compression gasses making it into the coolant, shouldn't that be enough pressure to push past the tstat even before temps are up?

Id verify with a pressure test first, confirm water pump flow and basically troubleshoot the cooling system. He should have been able to get it shut off before overheating it. Also, the 94 is pressure capped on the rad and just has an overflow bottle. A cap failure could quickly fill the bottle.
