Broken Pushrod after head gasket repair

Guys I have an 06 F350 6.0 with about 160,000 miles on it. About 1000 miles ago I replaced the head gaskets for a customer with arp studs. Egr delete and the such. Also installed full force 155cc injectors. Truck ran perfect for about 1000 miles. Customer called said truck was idling rough. Found a messed up broken pushrod in #7 cylinder. No valve to piston contact. Said it was perfect the night before, and his dad took the truck the next day and started hard and was running rough. Any reason this would have went? Confused as to what actually happened. have done many of these and could see no installation error.

Removed the high pressure oil pump to peek at the cam. Lobe gone. :jawdrop:

I have dealt with broken lifters a lot with 6.0s and 6.4s. At first I thought a fluke, then maybe lifter bore issue, but seeing how the lifter in these trucks has been the same as the first 6.9 diesel, and for most part lifters don't fail in idi, 7.3 di motors, and the failed ones I see are usually rear cylinders, I have to think oil pressure related.
I have seen many reasons why a 6.0 won't make much oil pressure, but the obvious are the stupid oil filters that come with a cap. Most are over an Inch shorter than oe, meaning the drain is wide open. With a 1/2 inch hole in pressure side, idle pressure may only be around 5 psi, just enough to open the switch so idiot gauge looks ok.
Every time I've seen a failed lifter, there has been issues in the oiling system.
