Trucks making a weird squealing grinding noise at idle and I'm thinking its my pulley tensioner up on the top right corner of the motor. How bad is the pulley tensioner to replace?

The belt tensioner is on the lower left if the engine. It's not too bad but routing the belt sucks.

Well it's the ac belt tensioner it blew out this morning. Belt is shot I'm going to have to fix it tomorrow at my buddy's place. I'm going to Florida on Friday with no a.c. And nowhere to buy parts.

Pull the top half of the fan shroud off. Most likely have to undo both hot and cold side intercooler boots to remove the shroud. Then if you can get the fan off ( left hand threads on it) it shouldn't be too awful after that. You will have to remove the regular belt tensioner and then you can get the AC tensioner off.

The worst part of the job is trying to get the belt between where the main belt tensioner bolts to/bracket and onto the compressor. At least IMO. But definitely a doable job without removing the fan.
