What causes injectors to hang open?

So I've been seeing quite a few threads where people are having injectors hang open causing cracked pistons. What is causing the injectors to hang open? Contaminated fuel? Hpfp going bad? My truck has 130,000 miles on it and this has me kinda concerned. Should I be looking into replacing injectors or hpfp in the near future?

Could be a lot of things, water causing rust in the fuel system, a filter that's coming apart allowing debris to enter the injectors, or a hpfp going bad and putting metal bits into to the fuel. the fuel system on the 6.4s is very sensitive doesn't take much to wipe out the entire system

I will be pulling all injectors this winter and have them bench tested since they have over 100k on them. I will also place my front pump before it decides to say uncle and ruin my whole system. Just throwing out some options for u. I would prefer to replace parts known to fail before they fail and it costs me at least 5k.

Where is a good place to have the injectors bench tested? I probably only put 300 miles per month on my truck, so down time to do some preventative maintenance isn't a big issue for me.
