Freeze plug blow out in head

I bought my Job 1 '08 F250 6.4L from an auction as-is and have no prior history of it. It had all the symptoms of blown head gaskets/cracked head, so we pulled the cab and tore it down. Absolutely no signs of damage to the gaskets or heads, but this freeze plug was out of place. Is this common?

not from what i have seen thats not common, Could have a reman head or it had a gasket failure making the cooling system pressure high

I've never seen one blow out, it could have been installed wrong or just happened to be the weakest one when the water in the engine froze.

Ok, I took the heads away for inspection, surfacing and valve job. How do I get all the oil/water sludge out of everything else?
