noticed awhile ago that my fuel wheels 18x12 have a valve stem on the inside and outside of the wheel/tire , i have no experience with fuel stuff so what does this mean? i have toyo open country AT 2 extreme.

My 20x12 fuels have two per wheel also. Only reason I could think of is easier access to the valve stem when seating the bead as a x12 wheel is pretty frigging big.

The 2nd valve stem is taking the place of where the tpms sensor would go for vehicles equipped with it.

Yes. It fills the inside of the wheel that doesn't normally get enough air when filling from the outside.

Be careful though, as there have been reports of the air disturbance causing a Vortex in the tire resulting in unexplained shimmies sometimes referred to as death wobble.

One is for summer air, and the other is for winter air. It keeps a perfect balance. Just make sure you have the same amount, or bad things will happen.
