Mile marker hubs?

for the 2nd time my new mile marker hub have came out of the axle and have been sitting inside the cover for the rim. Both times have been the drivers side.

Think its a bad hub or is there a different problem thats pushing these hubs out.

mine has been on for about 20,000 miles with no trouble.

I had that issue with warn premiums.turned out the wheel bearing was shot and the needle bearing where the stub axle shaft ride had seized up. Thus spinning the hub while it was disengaged.
