Hope that fixes it, I'm sure you already covered this but did you try wiggling the harness at the ficm the 3 connections?

My truck ran like complete crap after a turbo swap one day one of the plugs at the ficm had knocked loose during the process somehow.

ICP sensor is in the hpop cover....very common thing for them to go bad in the 03-early 04's.
